Stock Order Form

Healthcare professionals registered with the General Medical Council (GMC), General Dental Council (GDC) or General Pharmaceutical Council (GPhC) can order selected products without the need for a prescription by completing our Stock Order Form.
The Stock Order Form is available to download below and can also be accessed through your Revolve Medicare online portal.
Once completed, please email your form to info@revolvemedicare.co.uk. Our pharmacy team will review your submission and process your order as quickly as possible.
Before You Submit
To avoid delays, please ensure that:
✔ All mandatory fields are completed accurately.
✔ Your professional registration details are correct.
✔ Your CQC-registered premises are clearly stated.
✔ The form is signed where required.
Incomplete or inaccurate forms may delay the processing of your order.
Delivery Requirements
In accordance with UK regulatory requirements, all stock orders must be delivered to a CQC-registered premises. We are unable to dispatch stock orders to residential or non-registered addresses.
