Professionals registered with the General Medical Council (GMC), General Dental Council (GDC), or General Pharmaceutical Council (GPhC) may now download our Stock Order Form to place product orders without the need for prescriptions.
The Stock Order Form is available for download below and can also be accessed via your online portal. Once completed, please return the form to us by email at info@revolvemedicare.co.uk. Upon receipt, our team will review and process your order accordingly.
Please ensure that all mandatory fields are accurately completed, including details of your CQC-registered premises. Incomplete or inaccurate submissions may result in processing delays.
Kindly note that all orders must be delivered to a CQC-registered premises, in accordance with regulatory requirements.
